This Peer Evaluation form is designed to provide constructive feedback to your team members, fostering individual growth and improving overall team performance. The information gathered will be used to identify individual strengths and areas for development, improve team dynamics and collaboration, inform individual performance reviews, and recognize and celebrate contributions.
The form is divided into two sections and will take 5 - 10 minutes. Be professional, honest, and objective in your assessment, focusing on observable behaviors and their impact. Provide concrete examples to support your feedback. Your responses will be kept confidential, and the evaluee will not be able to identify who provided the feedback.
